Your Medical Records

Medical records are confidential. Therefore, we will not release any of your medical information without first obtaining a signed authorization from you. You may come into the office at any time to obtain and complete your Medical Records Request/ Authorization. However, please keep in mind that records are copied on Tuesdays only.

In addition, there is a fee of $.50 per page for the first 50 pages and then $.15 per page thereafter.
